The Haverstraw King's Daughters Public Library Teen Council is a regular volunteer group. The purpose of Teen Council (otherwise known as a Teen Advisory Board) is to provide local young adults with leadership, planning and interpersonal skills they can take with them into their future. Teen Council will also provide a crucial role in service and support of people and organizations in Haverstraw and neighboring areas.

Membership is open to teens 13 and up who live within the North Rockland Central School District.

By allowing my child to participate, I understand and acknowledge that in order to receive full credit for volunteer service hours, they must:

1. Attend a minimum of 6 monthly meetings over the course of the school year (September through May)
2. Be able to attend both onsite (at the library) and offsite (out of the library), local volunteering opportunities, with enough notice given ahead of time for logistics
3. Be a resident of the North Rockland Central School District.

By signing this form, I confirm that I have read and understood the terms outlined above.

I freely and voluntarily give my consent for my child to participate in the activity, program, or service described. I understand the responsibilities, requirements, and any potential risks involved.

I accept the terms and conditions of this agreement and agree to comply with all related rules and policies. I understand that I may withdraw my consent at any time by contacting the organization, recognizing that doing so may affect my ability to continue with the related service or activity.
