US Passports
Haverstraw King’s Daughters Public Library is a United States Passport Acceptance Facility, designated by the U.S. Department of State to accept and execute passport applications.

You must apply in person using Form DS-11 if at least one of the following is true:
• You are applying for your first U.S. passport
• You are under age 16
• Your previous U.S. passport was issued when you were under age 16
• Your previous U.S. passport was lost, stolen, or damaged
• Your previous U.S. passport was issued more than 15 years ago

2. Proof of citizenship. See the Citizenship Evidence section of the U.S. Department of State website, click here.
3. Proof of identity/photo identification. See the issued Photo Identification section of the U.S. Department of State website, click here.
4. One 2×2″ color passport photo. See Photo Requirements on the U.S. Department of State website, click here. We can take photos for you for a $10 fee at Haverstraw King’s Daughters Public Library. Please call to schedule.
5. Two checks or money orders per applicant.
THERE ARE TWO PAYMENTS
1. To the U.S. Department of State; a separate check is required for each applicant.
2. To the Haverstraw King’s Daughters Public Library for the Execution Fee.
All fees are set by the U.S. Department of State and are non-refundable. All passport acceptance facilities charge the same fees.
PASSPORT BOOK FEE
• This fee must be paid by check or money order payable to the “U.S. Department of State”
• Routine Processing (4-6 weeks):
⁃ Applicants age 16 and older: $130 per application
⁃ Applicants under age 16: $100 per application
• Expedited processing (2-3 weeks) is an additional $60 per application
•For overnight return of the passport from the U.S. Department of State, it is an additional $18.32
EXECUTION FEE
• $35 per application
• Cash, check or money order payable to “Haverstraw King’s Daughters Public Library.” Visa and MasterCard payments are also accepted.
• You may combine this payment on one check/money order/card payment for more than one application.
ADDITIONAL FEES
• Photos are available for $10. The cost will be added to the Execution Fee.
• Express overnight delivery of an application from Haverstraw Library to the U.S. Department of State is available for $29.20.
• The Passport card facilitates entry and expedites document processing at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda.
NOTE: The Passport Card CANNOT be used for air travel.
PASSPORT CARD FEE
This fee must be paid by check or money order payable to the “U.S. Department of State”
• Routine Processing (4-6 weeks):
• Applicants age 16 and older: $30
• Applicants under age 16: $15
• Expedited processing (2-3 weeks) is an additional $60.
PASSPORT CARD RENEWALS
A previous passport book holder, eligible to use the Renew by Mail Form DS-82, may apply for a Passport Card as a renewal. First time applicants for a U.S. Passport, and those not eligible to use Form DS-82, should apply for a Passport Card using Form DS-11, click here.
